Sliding Fee Scale Application
Coal Country Community Health Center is committed to providing affordable, quality health care to our entire community and surrounding area.
We offer an income-based discount program (sliding fee scale) to all qualifying patients. The sliding fee scale provides a discount on medical services including dental and prescription drugs. The program is available for all qualifying patients uninsured and underinsured and is applicable at Coal Country Community Health Centers only. The discount program is not an insurance plan. Please keep in mind that discounts are not valid on charges or appointments with a visiting provider. Other exclusions may apply.
The application process is quite simple, and an application is available for download below or by stopping in to the clinic.
